The church is located in the Staro Selo neighborhood, 2 km east of the center of Milanovo village. The church is partially dug-in at the north and east facade. It is single-nave, vaulted, single-apse with a proskomidia niche on the northeast wall and a washbasin on the north. The throne is stone with ornamental decoration. The roof is gable covered with large stone slabs. The length of the temple is 8.20 m., width - 5.15 m., height of the vault - 5 m. The wall construction is from mixed stones found in the vicinity, bonded with mortar. The thickness of the walls is 0.80 m. The floor is made of large stone slabs, the walls are covered with plaster and are painted. The frescoes date back to the 17th century. The temple has the status of “Artistic cultural value with preliminary category of National significance” since 1982.

    There is a patron niche above the entrance. The image within is almost covered by plaster applied much later. The founder's inscription located on the western wall above the entrance is very interesting. According to the inscription the name of the church is "Holy Nativity of the Most Holy Lady". The sign provides information that the church was built in 1492, but according to the research and iconographic study of Dr. Z. Zhdrakov the temple is from the 17th century, not the 15th century. According to Dr. Z. Zhdrakov, the text is unfinished. The entire church inside was painted with frescoes. There are separate fragments of them preserved nowadays, and a significant part was hidden under later plaster. In the apse a relatively well-preserved image of the Virgin Mary with a medallion on her chest of Christ - Emmanuel, and on both sides of her - two archangels are painted. А frieze with medallions of saints are situated below. There is a fragment of an image of a saint (behind the throne on the south wall) and a biblical scene (next to the west wall). A relatively well-preserved image of the Virgin Mary is at the zenith of the vault (in the western part). The best-preserved frescoes occupy a section of the vault (above the south wall), attached to the west wall. Fragments of the scenes "Entry into Jerusalem" and the entire scene "The Last Supper" have been preserved from the south along the base of the vault to the west. The iconostasis is simple - wooden, made relatively late (second half of the 19th century). The bishop's throne, attached to the south wall, also contributes to the interior design. The long years of problems with the roof have affected the frescoes. The vault and walls were wet, and in the winter the plaster froze, causing a significant part to be irretrievably destroyed. From the north, as a result of the high humidity and the processes of carbonization and sulfation, the painting was covered with a thick layer of salts, making it illegible and unclear. The church was restored in 2016-2017, the roof was repaired, and some frescoes that had previously remained hidden under thick layers of plaster and salts were revealed. The frescoes were cleaned, strengthened and preserved. On the western facade, the severely damaged frescoes were revealed and restored. To the left of the door is depicted St. Petka as a small figure, and next to her is the powerful figure of St. Archangel Michael - the guardian of the temple. In the thickness of the wall around the door, there are crosses with texts. The Equal-to-the-Apostles St. St. Constantine and Helena are depicted to the right of the door.
    The Feast of the Assumption of the Blessed Virgin Mary is celebrated annually on August 15 with a sacrifice on the meadow next to the church.
